As previously said, boys are meant to be bigger than girls, personally I think as long as he has a good mixed diet, has free range time and is healthy there shouldn't be a problem
I would think he is probably quite bored and depressed, so a cage mate might be a good way to go, though you will find it would be better to get one of the same ish age.
I have 2 boys who are 1 and 1 and a 5 months, they are very melow boys and I would suggest that you either look for a rat that is laid back or loko for someone who is willing to take him on as a cagemate for their own calm lad (ME ME ME)
Surgery may be a little drastic, esp if he is over weight

It's actually much easier to introduce young males to an adult male than it is to introduce two adult males to eachother.
