If you take some of the dirt off of him and put it in water it will go a red/brown colour that is because the blood the flea drinks from the kitten comes through in the feaces.
Vets always charge an arm and a leg for flea treatment. What did they give you Panacur or Frontline??? If they gave you frontline drops then he only needs done every 2 months.
Next time you need some go on the internet it's a lot cheaper. You can get wormer, Drontal, cheaper too.
Hyperdrug Equine Pharmacy - Hyperdrug, Home of the Equine Pharmacy, Canine Chemists and Pigeon Pharmacy, pet medicines, wormers, and flea treatments Is a really good vet chemist. They deliver and are really good.

The flea treatment will work but the dirt will be there until it is either brushed out or falls out of his fur. Don't try and wash it out as this could prevent the flea treatment working to it's full extent.
Fleas tend to 'congregate' on the head and around the base of the tail.
Did they put drops on his neck or use a spray???
Out of interest how much did they charge for the flea treatment?? it should show seperately on your receipt. They are supposed to itemise each treatment for you.

I must admit I clip all my cats claws every month or else I would'nt have any walls left.

But I do think vets just do things to charge more.

Freeky,

Had he already had his vaccs. Because if that £90 included vaccination then it wasn't too bad really.

Could be cat acne on his chin. Vet advises cleaning off.
Even with vaccinations £90 is very dear

I can see that - I recommend that any new kitten owners do it for a while (and show them how) to stop them getting caught up in curtains - but i'd be really p**d off if my vet just decided to do it, esp if the cat was just starting to go out - no defence!

If it's just on his chin and nowhere else, then it'll probably be chin acne - 4½ months - he's reaching adolescence! It's just excess grease coming out the pores. Cats get it on their chin, lips and entire males get it on the base of their tail (called "stud tail")

Try washing it with some gently shampoo and it that doesn't work then you can use a human prep like biactol (not in his mouth though!). If you look carefully in the hair you might see actual blackheads in the skin. Good for squeezing if that's your bag!!!

So do I, not every month, but whenever I feel them getting sharp! You have to do it if you show, because they don't want the judges scratched (poor judges!!!).
