Could cause an impaction and fatality in theory, i dont have any substrate, easy to clean, and safe.

either larger substrate, so it cant be swallowed or none at all. Which is actually quite nice once you get used to it, as it is incredibly easy to keep clean.
I cant sit here looking at my tank if i can see they have pooped.

I have a large slider - I use river sand rather than nothing. It means cleanning replacing filters more often but looks great and he is 11 yrs old now!

If the inlet of your filter is raised i think the use of sand doesn't really cause issues.
Anyways, the reason i use nothing is because its immensely easy to clean.
